Essentials At A Glance
Program Type: Community-based
To Make Referrals:
ICM: For institutional case management, referrals come directly from the Public Conservator’s office and/or coordinated through San Diego’s Department of Behavioral Health. This program only serves individuals that are on public conservatorship.
SBCM: For strength-based case management, referrals come from a variety of places including hospitals, other providers, family members, and individuals. All referrals must be submitted through Optum’s Single Point of Access (SPOA) by submitting an online referral form or calling 1-800-842-2652 to request strengths-based case management services.
# of Members: 354 ICM members; 352 SBCM members
Who We Serve: Adult residents of San Diego County, ages 60 and older, who are diagnosed with a serious mental illness. For ICM services, the individual must also be conserved and placed in a locked, residential facility.
Hours of Operation:
Monday - Friday: 8:00 am- 5:00 pm, evenings and weekends as needed
Crisis services are available 24/7

We believe that integrated healthcare and care coordination are key components for your recovery journey. Our job is to do whatever it takes to help you get the proper care you need and feel empowered through your choices of care. We have a full staff of peer specialists, case managers, and mental health rehabilitation specialists who are all here to help you on your path.
Your recovery plan is guided by your hopes and dreams. Our team will help you by providing personalized assessments where you can identify the kind of program assistance you will need. From there, we will collaborate primary care physicians and coordinate with home health agencies, senior centers, and other community resources for older adults to make sure you get the most comprehensive care possible.
