Essentials at a Glance

Program Type: Community-Based; Population Management

To Make Referrals: Referrals are coordinated through Kaiser Permanente.

Who We Serve: Members of Kaiser Permanente, 18 years and older, who have been diagnosed with a serious mental illness, who have had previous patterns of high inpatient care utilization or otherwise be at high-risk for readmission.

Hours of Operation: Open Monday - Friday: 8:30 a.m. to 5:00 p.m. Crisis services are available to members 24/7.

About the Program: The Kaiser Intensive Community Treatment (ICT) program provides specialty mental health care services that partners with you and your Kaiser Permanente wellness team to achieve a state of wellness and quality of life that is meaningful for you.

We have a diverse team of clinicians, substance use specialists, and peer staff that will work with you every step of the way to resolve your current crisis in ways that support your goals in life. Our team of recovery-centered providers also includes Spanish speaking team members.
