About MISSION POSSIBLE: The MISSION POSSIBLE team was developed to provide specialized, recovery-focused services in the community using mobile vehicles to serve individuals with serious mental illness who are homeless in Orange County, CA.
Our model of care focuses on multidisciplinary teams that will build relationships with individuals leading to successful linkage with ongoing mental health services, physical healthcare, and housing. The Outreach Team consists of two case managers and a peer support specialist. The Treatment Team has a nurse practitioner, two case managers, and a peer support specialist. We work with the Orange County Health Care Agency Outreach and Engagement Team to provide regularly scheduled visits within the county and as permitted by local jurisdictions so that clients come to know when to expect us.
The Mobile Outreach and Treatment Services are funded by the Homeless Mentally Ill Outreach and Treatment grant.
